Dual Agonist Therapy for Chronic Weight Management
Zepbound is a once-weekly injectable prescription medicine. It is a dual agonist, mimicking two hormones (GLP-1 and GIP) to regulate appetite and improve how your body processes sugar and fat.
1. What is Zepbound?
- Indication: For adults with obesity (BMI ≥ 30) or overweight (BMI ≥ 27) with weight-related medical conditions (e.g., high blood pressure, Type 2 diabetes, or sleep apnea).
- Usage: Must be used in combination with a reduced-calorie diet and increased physical activity.
2. Expected Weight Loss Results
In clinical trials (SURMOUNT studies), Zepbound showed significant weight loss results over 72 weeks:

3. How to Use the Injection
- Schedule: Inject once weekly on the same day each week, at any time of day.
- Administration: Subcutaneous injection in the abdomen, thigh, or upper arm. Rotate sites weekly.
- Meals: Can be taken with or without food.
- Storage: Store pens in the refrigerator. Pens can stay at room temperature for up to 21 days if necessary.
4. Dosage Schedule
Your doctor will use a step-up approach to help your body adjust:
- Month 1: 2.5 mg weekly (Starting dose; not for weight loss).
- Month 2: 5 mg weekly.
- Increases: Dose may increase by 2.5 mg every 4 weeks until reaching your maintenance dose (up to 15 mg).
5. Common Side Effects
- Nausea and vomiting
- Diarrhea or constipation
- Stomach pain or indigestion
- Fatigue and temporary hair loss
6. Important Safety Considerations
Boxed Warning: Thyroid Tumors Zepbound caused thyroid tumors in animal studies. It is unknown if it causes these in humans.
- Do not use if you or a family member have ever had Medullary Thyroid Carcinoma (MTC) or MEN 2.
- Report any neck lumps, trouble swallowing, or hoarseness immediately.
Other Serious Warnings:
- Severe Gastrointestinal Disease: Zepbound is not recommended for people with severe gastroparesis (stomach paralysis).
- Birth Control Warning: Zepbound may make oral birth control pills less effective. If you use the pill, talk to your doctor about switching to a non-oral method or adding a barrier method (like condoms) for 4 weeks after starting and after each dose increase.
- Pancreatitis: Seek help for severe abdominal pain that doesn’t go away.
- Kidney Issues: Severe dehydration from vomiting can lead to kidney damage.
7. Zepbound vs. Wegovy
Mechanism: Wegovy mimics one hormone (GLP-1); Zepbound mimics two (GLP-1 & GIP).
Efficacy: In trials, Zepbound generally led to higher average weight loss (20.2% vs 13.7% for Wegovy).
